I, the undersigned, am the parent or guardian of a participant in the 2025 Youth Leadership Camp. I hereby release Mississippi Families for Kids, its Board of Directors, sponsors, Christ United Jackson, camp presenters, speakers and all agents associated with said camp from all claims arising out of my child’s participation in said camp for damage to the property of my child and/or to the property of others as a result of the acts of my child. My child is aware that the director(s) and instructors will be in charge of the participants, and the participants must follow their directions. I understand and agree that Mississippi Families for Kids, its Board of Directors, sponsors, Christ United Jackson, camp presenters, speakers and all agents associated with said camp shall not be liable for injuries from any and all claims, damages or expenses in the event any suit is commenced by or on behalf of my child.
Medical Release
I, the undersigned certify that my child is physically fit to participate in the camp. By signing this form, I, the parent or guardian, do hereby release Mississippi Families For Kids and all connected persons or organizations with said camp from any and all liability or medical claims my child may incur while in attendance at said camp. However, the camp staff has my permission to seek medical attention for my child should the need arise.
Photographic and Video Release
I hereby consent to and authorize the use and reproduction by Mississippi Families For Kids, or anyone authorized by Mississippi Families For Kids, of any and all photographs or video of my child(ren) . Mississippi Families For Kids reserves the right to use photographs and video in any of its print, media or electronic publications.
I hereby acknowledge that I have read and understood the terms and of these releases.
